Patients with chronic heart failure experience a significant disease burden and are often faced with navigating a complex health care system, in addition to bearing the added burden of adverse social determinants of health (SDoH). It is well known that SDoH accounts for determining 80% of health outcomes1 and that identifying and addressing SDoH improves health outcomes for patients with heart failure.
